Empty line at EOB

Hi folks,

I've just committed a changeset f45a6d9 that removes the
final-line-adjustment from evil.

This also implied some other changes because now some commands have to
deal explicitly with the last (empty) line in buffer (formerly, this
line could not be selected so it could never be covered by a motion
passed to an operator, in some sense).

Of course, this change could introduce new bugs. Please report them
using the issue-tracker.

Some of the open issues are directly related to this problem. Please
re-check if they are valid anymore and close them if not. If you find
new bugs that are not directly related to an old issue, please close
the old issue and open a new one (so we can keep track of the issues).
