Greetings.

It's been a long time since the last version of our paper. I am sorry (and responsible) for the delay. Now I have a compiled a new version that I would like to ask you to review.

I have tried to forge the chapters into one piece. So some of your paragraphs and sentences are now mingling with those of other co-authors. Please check if the content still makes sense. Some sections should be bolstered by references. Considering the length of the manuscript, you should rather delete than add when you work on the manuscript.

I have made the document available on google docs so we can see each other's comments and changes. I am also attaching a Word file for those who prefer working offline.

If you could work on the manuscript within the next four weeks that would be great.
